MSNBC’s Rita Cosby reports that Anna Nicole may have had recent breast surgery to repair one of her implants.

Cosby also reports the former Playmate had also been prescribed over 10 different types of pain medication, and may have been taking excessive amounts because she developed a tolerance to the effects.

In addition, Cosby’s sources tell her that Anna Nicole was found by a member of her staff — unresponsive and nude in her bed, with a sheet pulled over her face.

Death from cosmetic plastic surgeries occurs in one out of 57,000 cases, per the American Society of Plastic Surgeons. Two million plastic surgeries were performed in the United States in 2005.

Adverse reactions to surgery could become serious; there are numerous documented cases of seizures, heart attacks and high fevers leading to death. Recent published research shows that inflammation caused by common respiratory and urinary tract infections like bronchitis, pneumonia and cystitis, may contribute to in increased risk of heart attack and stroke, and the greatest risk is in the earliest stages of infection.

If Anna did have a recent cosmetic procedure, and she exhibited “flu like symptoms” over the last few days, it’s quite possible that the star died of a massive infection, perhaps complicated by an accidental overdose of prescription drugs.

TMZ has obtained a photo of the inside of Anna Nicole Smith’s refrigerator taken after she died, and the contents are shocking on several levels. TMZ has 100% verified the authenticity of this photo.

The refrigerator, which was inside Anna Nicole’s bedroom in the Bahamas, contained methadone (5 mg per 5 ml). The small refrigerator contained cans of Slim-Fast– ironic, since Smith was TRIMSPA’s spokesperson. Several bottles of TRIMSPA sat right next to the fridge.

The fridge also contained Miracle 2000 (marketed as a nutritional supplement for “today’s active lifestyle”), French’s Worcestershire Sauce, yogurt and spray butter. It also appears there are vials of injectable medicines.

The methadone is compelling, as Smith’s son Daniel died from a combination of drugs that included methadone. Law enforcement sources in the Bahamas tell TMZ that at least one witness has come forward and said Howard K. Stern supplied Daniel with methadone just before he died, and flushed the remaining stash down the toilet. That witness is expected to testify next month at the coroner’s inquest into Daniel’s death.

Now Stern is placed in the same home with another stash of methadone — the home he shared with Smith.

How did she die?
Unknown. An autopsy Friday and a toxicology test will ultimately decide cause of death. Smith had battled health problems recently and there was a nurse in the hotel room with her. It’s not known why the nurse was there, but Smith was hospitalized in November for pneumonia and suffered flu-like symptoms before her death. Also, Cyril Wecht, the Smith-hired pathologist who believed Smith’s son died of a combination of medication and a heart condition, says that Smith could have had the same heart problem.

What’s the status of the inheritance?
It had been looking good for Smith. She claimed her late husband, oil billionaire husband J. Howard Marshall II, promised her millions. After he died in 1995, she tangled with Marshall’s son. A California bankruptcy judge awarded Smith $474 million, later overruled by a federal judge who reduced it to $88 million. An appeals court then ruled she was entitled to nothing because the federal courts lacked jurisdiction in a matter that should be handled in Texas state court. But the Supreme Court recently overturned that decision, and allowed lawyers to go the next round in federal court, which is where the case stands.

Would Dannielynn get the inheritance?
Unknown. As Smith’s only surviving child – son Daniel died three days after Danielynn’s birth – the daughter is theoretically entitled to most if not nearly all of Smith’s estate, including any inheritance. Smith’s death, however, throws the already tangled inheritance fight into chaos. Also, it’s still unknown whether Smith had a will, and, if so, what provisions she made for her daughter. If she died without a will, by law everything would go to Dannielynn. The matter likely will be tied up in courts for years.

Were Howard K. Stern and Anna Nicole Smith ever legally married?
No. They had what they called a “commitment ceremony” on Sept. 28 in the Bahamas, but it was not legally binding under Bahamian or American law. “They were partners, soul mates,” says Smith’s lawyer.

*Narcotics found in Anna’s hotel room. Suspicion that drugs killed Anna Nicole Smith was heightened today as investigators found illegal narcotics as well as prescription medications in Anna Nicole Smith’s room at the Hard Rock Hotel in Hollywood, Fla., according to reports this morning.

WCBS-TV in New York reports that drugs were found in Smith’s hotel room, and a toxicology report that would determine whether those drugs caused her death has not yet been released. An autopsy is scheduled for this morning.

*Did Anna choke on her on vomit?  According to another report this morning, when an “employee” of Anna Nicole Smith found her in her hotel room, she appeared to have choked on her own vomit. The “credible source” cited by Access Hollywood says that when the employee encountered Smith in her room, she pulled back a sheet and discovered that the model-actress had choked. The same source, says Access, claims that Smith took a “children’s sedative” and that she “kept passing out” before her death.

*Smith’s Final Moments Sell for $500,000. The footage of Anna Nicole Smith’s last moments sold for more than $500,000, just hours after Smith’s death in Hollywood, Fla., according to Splash News & Picture Agency, as reported by FOX News. The two-minute video, according to sources, shows paramedics working on the unresponsive actress, and her body being wheeled out on a gurney. The paramedics, according to a producer who viewed it, appear to pull the gurney back out of the ambulance to administer CPR again. But, adds the producer, the body is not readily identifiable as Anna Nicole Smith.

The two-minute tape, shot from three different angles from a distance of about 50 feet, shows Smith’s sheet-covered body, with an oxygen mask over her face, being taken out of the Hard Rock Hotel by a gaggle of emergency responders, including paramedics and police. “You cannot see her face, you cannot see hair flowing out from underneath the mask,” Spinder said. “Again if someone didn’t tell me this was Anna Nicole Smith, I totally would not have known it. You can’t even tell it’s a woman.”The video shows footage of the ambulance driving away from the hotel under a police escort, then cuts to the ambulance at Memorial Regional Hospital just after 2 p.m. ET Thursday. “You do not see the ambulance pull into the hospital, and you do not see the body come out of the back of the ambulance,” Spinder said. “You just see the ambulance sitting there.“One would assume that the cameraman got there after the body had already been unloaded from the ambulance and was already in the hospital at that point,” he said.

*Anna’s mom said custody battle “Will get ugly.” The mother of the late Anna Nicole Smith predicted that the fight over her granddaughter Dannielynn would get “ugly,” and dismissed Anna Nicole’s rags-to-riches self-portrayal as mere myth-making.

In an interview on “Good Morning America,” a disconsolate, almost disgusted Virgie Arthur described Anna Nicole as “wild and crazy,” and reiterated that she’d warned her daughter to stop using drugs. She added that she “really sincerely” worries about five-month-old Dannielynn, and wants her to be with her father — whoever that might be. When asked about the pending paternity battle, Arthur agreed that it would get ugly.

Curiously, Arthur also took the opportunity to debunk the myth that Anna Nicole’s rise to fame was a classic rags-to-riches tale. “Why do you tell people stuff like that?” Arthur said she asked Anna Nicole. “I’ll do whatever it takes,” replied Anna Nicole. “If my name’s out there, I make money.”

(3:25PM)*Anna Nicole Moved to VIP Area. Sources inside Memorial Regional tell TMZ that Anna Nicole’s body has been moved to a VIP section of the hospital, which is off-limits to everyone except for key hospital personnel and requires special keycards for entry.

A death certificate will be filed within 24 hours.

*Attorney said she suffered from “flu like” symptoms this week. Attorney Ron Rale says Anna had been under the weather with “flu-like symptoms” the past week and she “showed signs of a fever” last night. According to Rale, Anna’s lawyer-turned-partner Howard K.Stern is at the Miami hospital now and is overwhelmingly distraught.

*Attorney said that DNA will be discussed in court Friday.  Attorney Ron Rale, who had been representing Anna Nicole in her paternity battle with Larry Birkhead, tells TMZ there will be a court hearing tomorrow. He says he was contacted by reps for Larry Birkhead and they will go to court to discuss the matter of DNA testing.

*Trimspa releases a statement. TRIMSPA has released the following statement regarding Anna Nicole’s death: “Today, Anna Nicole Smith’s grief stricken and tumultuous personal life came to an end. Anna came to our Company as a customer, but she departs it as a friend. While life for Anna Nicole was not easy these past few months, she held dear her husband, Howard K. Stern, her daughter, Dannielynn Hope, her most cherished friends, beloved dogs, and finally, her work with TRIMSPA. Anna knew both the joy of giving life, and the heartache of losing a child. We pray that she is granted the peace that eluded her more recent days on earth, and that she finds comfort in the presence of her son, Daniel.”

*Paramedic said that Anna was dead when he got to her hotel room. NBC affiliate WTVJ in Miami did an on air interview with a Hollywood, Florida paramedic who was one of the first to arrive at Anna’s hotel room after she was found unresponsive. The paramedic said she was not alive when he got to the room.

(3:37PM)*Sources say that Hospital Personell believe Anna died from heart failure. How appropriate yet tragic that would be.

*Howard K. Stern IS Danielynn’s legal guardian. With the sudden death of Anna Nicole Smith, Howard K. Stern has already assumed the title of “legal guardian” of Anna Nicole Smith’s 5-month-old daughter Danielynn, according to the laws of the Commonwealth of the Bahamas.

The law states, “On the death of the mother of a child, the father, if surviving, shall, subject to the provision of this Act, be guardian of the child, either alone or jointly with any guardian appointed by the mother. ”

Smith named Stern as the father on Danielynn’s Bahamian birth certificate.

(4:08PM) *SPLASH NEWS posts this picture on their site, claims to have video of CPR being performed on an unresponsive Anna Nicole. I really REALLY hope this isn’t true…but it looks legit to me.

*Anna Nicole’s former lawyer said she struggled with alcohol. Lenard Leeds said, “Anna always had problems with her weight going up and down, and there’s no question she used alcohol.” Leeds says it’s no secret that, “she had a very troubled life” adding Anna had, “so many, many problems.”

The former playmate often drew comparisons to another blonde bombshell, Marilyn Monroe. Leeds said, “She wanted to be like Marilyn her whole life and ironically died in a similar manner.” Marilyn died at age 36 in 1962.

During Leeds’ years working for Anna, “she embraced all the media attention, but later in life it became her worse enemy.”

*Anna’s former publicist say’s he’s not suprised.  David Granoff, Anna’s former publicist, told MSNBC that he was “sad, but not shocked” by Anna’s death. He added that there was “Just no spark in her anymore.”

*Autopsy to be performed tomorrow (Friday).

*Anna’s daughter was not with her. People.com is reporting that Anna’s 5-month-old daughter, Dannielynn Hope, was not with her at the time. Sources tell the magazine that she is being cared for in the Bahamas by the mother of Shane Gibson, a high-ranking Bahamian official who is a close friend of Smith’s.

TMZ has also learned that the hotel room that Anna was occupying at the Seminole Hard Rock Hotel and Casino will be sealed off as a part of the investigation.

*Judge in paternitey case had ruled that Anna must submit Daniel’lynn to a DNA test by February 21st. Is this a coincidence? The judge made this ruling just yesterday.
